For over 20 years, our MassHealth-funded program has supported older adults and individuals over the age of 16 who have a disability and live with a dedicated caregiver. We provide both health and financial resources to ensure safe, stable, and meaningful community living for our clients and their families.

What is Adult Foster Care?
Paid Home Caregiving
Adult Foster Care (AFC) is a MassHealth-funded program that supports individuals age 16 and older who have disabilities and live with a caregiver who provides assistance with daily living activities. It allows eligible adults to live at home with their caregivers, (most often family members), who provide the 24-hour care and support the individual needs. The caregiver receives a tax-free stipend, up to $1,700/month and support from a dedicated care team.

In addition to a tax-free monthly stipend, our caregivers benefit from personalized care plans designed to meet each individual’s unique needs. Every care team includes a nurse and a care manager who make regular visits to provide health and emotional support, help navigate local and statewide resources, and ensure continuity of care.
